Is there a subscription?
No. $29.99 once, updates included.

Not through a server. Because it is a folder of plain files, iCloud Drive, Dropbox or git will sync it if you choose so yourself.
Is it on the Mac App Store?
Yes, you can download it from AppStore.
Does it send my writing anywhere?
No. There is no network code in the app. Even the grammar checker works fully local.
Can I use my own Markdown files?
